Intervention Assistance Team reviews data from RTI (if applicable) and makes referral for evaluation for suspected disability.  At this meeting the referring party provides the completed PR04 to be accepted and signed by the team chair.

Within 30 days, the PR01 must be sent by the team chair to the parent indicating the determination of action to be taken by the team.  This can be included as a part of Step 1. PR 01  Prior Written Notice

If the team proposes an initial evaluation, the parent needs to be invited to a meeting to develop an evaluation plan and sign permission.  This step can be included as part of Step 1.

The date is arranged for the ETM, and PR 01 and PR03 are sent.  The ETM should be approximately 50 days from receipt of parent permission.  This allows time for reschedules before the 60 day time limit expires.

The evaluation team is notified of their roles and the ETR forms are completed, other than summary and determination.  PR 06  ETR

The ETM is held to discuss evaluation results.  At the meeting, the team completes a Summary (Part 2) and participants indicate whether they agree of disagree with the determination (Part 5) PR 06  ETR

If the student is eligible for services, parents are sent a copy of the ETR and an invitation to the IEP meeting to be scheduled within 20 days of ETM (to allow for rescheduling).

If parent signs the IEP, Prior Written Notice is not needed.  If parent does not sign or disagrees, then PR01 must be given to the parent.  If parent does agree, parent is offered a copy of Procedural Safeguards (PR 03) and a copy of the IEP at the meeting.  Copies of the IEP must be provided to the parent within 30 days of the meeting.
